• When a request for vision or hearing assistance is received, the member will refer the request to the President, Secretary or in his/her absence, to another Officer.

• The Officer will submit a Benevolence Questionnaire to the person wishing the assistance, noting the date on which the request was received. (This can be sent by email as well.)

• Upon receiving the completed questionnaire, the President will review the questionnaire for merit, and turn it over to the Benevolence Committee for review.

• The Benevolence Committee will view the request for completeness, contact the requester if more information is necessary, and arrive at a recommendation to present to the Board.

• The Board will investigate the possibility of a cooperative effort with other Lions' Club or LCIF.

• The Board will make the final decision as to the merit and amount of money to be donated on behalf of a given individual/family.

• Upon approval of the Board, the Treasurer will send a check.

• It would be the recommendation of this committee that the Appleton Noon Lions continue to focus on eye and hearing requests.
