Appleton Noon Lions Club Site Survey

The form below asks you to give your feelings about the 5 sites the Club visited during the month of March. For your benefit, we ask that you indicate your feelings about each site by placing a grade of 1 to 5 (5 being the highest) in each box, comparing - for example the food at one site against another. If you find that difficult, you can give more than one site the same grade (or even all 5's, if you wish).
